We’re proud to share that Johanna Cazares, Chief Growth Officer at Alcar Health, will be moderating a featured session at the NACHC Workforce Conference on October 21, 2025, in Las Vegas.
Her session, Beyond One-Size-Fits-All: How Two Care Models Designed Tailored Outreach and Enrollment Strategies for Aging and Underserved Populations, will explore how FQHCs and PACE programs can better serve diverse aging populations while aligning patients with the right coverage and care.

Together, they’ll share how culturally aligned outreach, bilingual education, and data-driven workflows have reduced uninsured rates and built sustainable enrollment strategies for aging and underserved populations.

We’re proud to share that Annalissa Santoemma, Population Health Consultant at Alcar Health, will be presenting at the Nevada Primary Care Association Annual Health Care Conference on September 10, 2025 in Reno, Nevada.
Her session, Optimizing Community Health Worker (CHW) Impact: Strategies for Success, will dive into what it truly takes to make CHW programs successful long-term — from supporting staff and engaging patients to aligning with value-based care.
Drawing directly from field experience, Annalissa will share practical strategies for:
Training and integrating CHWs into care teams
Using data to identify and prioritize high-risk patients
Building sustainable, impactful CHW programs that improve health equity
